Africa on the Move: The Role of Political Will and Commitment in Improving Access to Family Planning

Despite commitments to the program of action for the 1994 International Conference on Population and Development and Millennium Development Goal 5 (focused on maternal and reproductive health), little progress has been made in improving access to family planning and slowing rapid population growth in Africa. Lack of political will has been highlighted among the key factors behind the lackluster performance in addressing these sensitive development issues. However, the situation is changing with some African governments embracing family planning as a key tool for improving child and maternal health, slowing population growth, preserving the environment, and enhancing broader efforts to alleviate poverty.
This study examines factors that have propelled the change in attitudes of some political leaders to champion family planning, assesses how such political will has manifested in different contexts, and explores how political will affects the policy and program environment. Mixed policy analysis methods were employed, including desk review of policy and program documents and stakeholder interviews conducted in Ethiopia, Malawi, and Rwanda – three countries that have made phenomenal progress in increasing contraceptive use in the recent past.
Lessons from this study will help galvanize efforts to improve access to reproductive health services in countries where little progress is being made. The results provide useful insights on the dawn of a new Africa where strategic political leadership is playing an increasingly valuable role in overcoming the continent’s longstanding development shackles. The study shows that political will is mainly changing due to increased availability of evidence showing that high population growth undermines efforts to alleviate poverty and hunger as well as investments in the quality human capital that least development countries desperately need in order to transform their economies.
The high sensitivity about childbearing and suspicions regarding the intentions of western development partners in promoting family planning in order to slow population growth are dissipating as more Africans are opting to have fewer children and demanding family planning. This study points to the need for global development partners to be much more cognizant of the drivers of Africa’s emerging success and focus their development assistance on enhancing, nurturing, and highlighting local leadership traits, capacities, and systems that are producing positive results, as well as support governments that have embraced family planning to ensure that no woman has an unwanted pregnancy due to lack of family planning.

Food Security in a Climate-Altered Future, Part Two: Population Projections Are Not Destiny

Too often, discussions about future food security make only a passing reference to population growth. It is frequently framed as an inevitable force, a foregone conclusion – and a single future number is reported as gospel: nine billion people in 2050. But adhering to a single path of future population growth misses the opportunity to think more holistically about food security challenges and solutions. Several recent food security reports illustrate this oft-overlooked issue.
Accounting for Population a Challenge
Oxfam International’s Growing a Better Future: Food Justice in a Resource-Constrained World is a thorough and fascinating examination of failures in our current food system and future challenges related to production, equity, and resilience. It reports newly-commissioned research, carried out by modelers at the Institute of Development Studies, to assess future agricultural productivity and food prices given the anticipated impacts of climate change.
But both the modeling work and the report text utilize a single projection for population growth: the UN’s 2008 medium variant projection of 9.1 billion by 2050 (which has since been revised by the UN up to 9.3 billion). Early on, the report does recognize some degree of uncertainty about this number: “Greater investment in solutions that increase women’s empowerment and security – by improving access to education and health care in particular – will slow population growth and achieve stabilization at a lower level.” But such investments do not appear in the report’s overall recommendations or Oxfam’s food security agenda. This is perhaps a missed opportunity, since the range of possibilities for future population growth is wide: the UN’s low variant for 2050 is 8.1 billion, and the high variant is 10.6 billion.
Food Security, Farming, and Climate Change to 2050: Scenarios, Results, and Policy Options is another frequently-cited report published by the International Food Policy Research Institute in 2010. In recognizing that economic growth and demographic change have important implications for future food security, IFPRI researchers modeled multiple scenarios for the future: an “optimistic” scenario which embodies high GDP growth and low population growth, a “pessimistic” scenario with low GDP growth and high population growth, and a “baseline” scenario which incorporates moderate GDP growth and the UN’s medium population projection. Each of these scenarios was then combined with five different climate change scenarios to better understand a range of possible futures.
Using different socioeconomic scenarios enabled researchers to better understand the significance of socioeconomic variables for future food security outcomes. The first key message from the report is that “broad-based economic development is central to improvements in human well-being, including sustainable food security and resilience to climate change.” This focus on economic development is based in part on the “optimistic scenario,” which counts on high GDP and low population growth (translating to high rates of per capita GDP growth).
Unfortunately, the socioeconomic scenario construction for this analysis doesn’t allow for an independent assessment of the significance of slower population growth, since high population growth is paired only with low GDP and lower population growth is paired only with high GDP. Therefore, none of the report’s recommendations includes reference to reproductive health, women’s empowerment, or other interventions that would contribute directly to a slower population growth path.
Expanding the Conversation to Better Inform Policy
Without a more nuanced treatment of population projections in technical analysis and popular reporting on food security, decision-makers in the realm of food security may not be exposed to the idea that population growth, a factor so critical in many areas where food security is already a challenge, is a phenomenon that is responsive to policy and programmatic interventions – interventions that are based on human rights and connected to well-established and accepted development goals.
There are some positive signs that this conversation is evolving. A new climate change, food security, and population model developed by the Futures Group enables policymakers and program managers to quickly and easily assess the impact of slower population growth on a country’s future food requirements and rates of childhood malnutrition. In the case of Ethiopia, for example, the model demonstrates that by 2050, a slower population growth path would make up for the caloric shortfall that is likely to arise from the impact of climate change on agriculture and would cut in half the number of underweight children.
And recently, we’ve begun to see some of this more nuanced treatment of population, family planning, and food security linkages in a riveting, year-long reporting series (though perhaps unfortunately named), Food for 9 Billion, a collaboration between American Public Media’s Marketplace, Homelands Productions, the Center for Investigative Reporting, and PBS NewsHour.
In January, reporter Sam Eaton highlighted the success of integrated population-health-environment programs in the Philippines, such as those initiated by PATH Foundation Philippines, that are seeing great success in delivering community-based programs that promote food security through a combination of fisheries management and family planning service delivery. Reporting from the Philippines in an in-depth piece for PBS NewsHour, Eaton concluded:So maybe solving the world’s food problem isn’t just about solving the world’s food problem. It’s also about giving women the tools they want, so they can make the decisions they want – here in the world’s poorest places.
Making clear connections of this nature between population issues and the most pressing challenges of our day may be the missing link that will help to mobilize the political will and financial resources to finally fully meet women’s needs for family planning around the world – an effort that, if started today, can have ongoing benefits that will become only more significant over time. Integrating reproductive health services into food security programs and strategies is an important start.
Back in Malawi, just before we turned off the highway to the Lilongwe airport, I asked the taxi driver to pull over in front of a big billboard. We both smiled as we looked at the huge government-sponsored image of a woman embracing an infant. The billboard proclaimed: “No woman should die while giving life. Everyone has a role to play.” Exactly. The reproductive health services that save women’s lives are the same services that can slow population growth and bring food security within closer reach. -

Delivering Solutions: Advancing Dialogue to Improve Maternal Health
“Throughout the 2009-2011 Advancing Dialogue on Maternal Health lecture series, we always heard the same good news: we know how to save the lives of women and girls. But more political will is needed,” said Calyn Ostrowksi, program associate for the Wilson Center’s Global Health Initiative on December 15 for the launch of the series’ culminating report, Delivering Solutions: Advancing Dialogue To Improve Maternal Health.MORE
Joining Ostrowski were co-author Margaret Greene, director of GreeneWorks; Luc de Bernis, senior advisor on maternal health at the UN Population Fund; Tim Thomas, interim director for the Maternal Health Task Force; and Chaacha Mwita, director of communications at the African Population and Health Research Center.
One of the few forums dedicated to maternal health, the series brought together senior-level policymakers, academic researchers, members of the media, and NGO workers from the United States and abroad. The series consisted of 21 separate events, with hundreds of experts sharing their experiences and thousands of participants and stakeholders providing their expertise. The final report captures, analyzes, and synthesizes the strategies and recommendations that emerged from the series.
Promoting Social Change
Unlike other health issues, said Green during her presentation on the findings of Delivering Solutions, the field of maternal health requires a holistic and multi-faceted approach; that is, an approach that looks not only at health systems, but also at underlying social factors. The report divides maternal health into three broad categories: social, economic, and cultural factors; health systems factors; and research/data demands.
Looking first at the social, cultural, and economic issues, Greene highlighted the need to improve nutrition and educational opportunities for young women in developing countries. Policymakers must be convinced that investing in women is not just good for women but good for families and children, she said. The participation of male partners and other male family members is also needed to increase access to maternal health services, such as family planning, and promote gender equality. The report pointed to a number of recommendations to promote male engagement:- Target interventions that educate men about danger signs and pregnancy complications.
- Address pressures that many young married men feel to prove their fertility.
- Inform men about sexual rights and how they relate to the health and wellbeing of their partners.
Health systems and medical resources play an equally pivotal role in reducing maternal mortality as social factors. The report highlights several key areas for strengthening the health system including the expansion of healthcare workers, health finance schemes, technology, and commodity distribution.
One key recommendation is to integrate reproductive health and maternal health supply chains. Four key medicines, oxytocin misoprostol, magnesium sulfate, and manual vacuum aspirators, target the three leading causes of maternal mortality (post-partum hemorrhage, obstructed labor, and unsafe abortion). Efforts to improve the distribution of these commodities should be more widely dispersed in developing countries and supported by community-based interventions. Women in urban slums, for example, face unique challenges that are not being adequately addressed.
Additionally, new technologies should be more creatively and effectively used, in particular the use of mobile phones in rural communities.
Many of the policy recommendations offered by the report, as Greene pointed out, are low-cost and highly effective. Yet three significant challenges remain for the field in general:- Six countries – Afghanistan, Democratic Republic of Congo, Ethiopia, India, Nigeria, and Pakistan – account for over half of the maternal deaths worldwide. The unique problems of each of these countries must be addressed and solved.
- Integration of maternal health with existing health services along with an over-reliance on community health workers can overburden weak infrastructure.
- Unnecessary cesarean births are on the rise as more women deliver in private sector facilities. These births cost 2 to 18 times as much as vaginal births and create unnecessary risks for mothers.
Chaacha Mwita of the African Population and Health Research Center (APHRC), located in Nairobi has seen firsthand the result of an overburdened and inadequate maternal health system in both his personal and professional life. Mwita endorsed the findings of the series report, emphasizing in particular the focus on transportation systems, male involvement, stakeholder dialogue, and education.
Mwita said that collaboration at all levels is the key to improving maternal health. Policymakers must communicate with researchers, who, in turn, must communicate with doctors, nurses, and hospital administrators in the field. The collaborative in-country dialogue series between the Wilson Center and APHRC, he believes, was a highly useful and easily replicable way of encouraging dialogue among relevant stakeholders in the field.
The Big Picture
”Our hope is that we’ve been able to seed discussions,” said Tim Thomas of the Maternal Health Task Force, one of the co-sponsors of the maternal health series. “We hope those seeds will take root and flourish.” Luc de Bernis, senior maternal health advisor of UNFPA, echoed Thomas’ sentiments, emphasizing the need for continued dialogue.
While maternal health has drawn increased international attention, creating political agreement among policymakers is a complex and often difficult process. There has been marked, though uneven, progress in improving maternal health across the globe, but more must be done. The Delivering Solutions report provides a state of the field assessment as well recommendations for existing, easy-to-implement solutions.

The Hungry Planet: Global Food Scarcity in the 21st Century

At the dawn of the 20th century, the world population was inching toward a modest two billion. In the 111 years since, notwithstanding the impact of war, genocide, disease, and famine, the global population has soared, reaching three billion around 1960 and now quickly approaching the neighborhood of seven billion. By 2050, the planet will likely be home to two billion more.
We may not be witnessing the detonation of the “population bomb” that Paul Ehrlich warned of in his seminal 1968 book, but such rapid demographic change is clearly pushing the international community into uncharted territory. With a limited amount of arable land and a finite supply of fresh water for irrigation, figuring out how to feed a planet adding upward of 70 million people each year looms as one of the 21st century’s most pressing challenges.
The push to ensure global food security transcends the desire to avoid repeating the famines that devastated the Soviet Union, China, North Korea, Ethiopia, and so many other corners of the world during the past century. Instead, aid and development organizations today rightly view food insecurity problems as deeply intertwined with issues of economic development, public health, and political stability, particularly in the developing world. To maintain order in the international community and prevent the emergence of new failed states in the decades ahead, it will be critical to find innovative means of feeding the rapidly growing populations of sub-Saharan Africa, the Middle East, and South and East Asia.

Connections Between Climate and Stability: Lessons From Asia and Africa
“We, alongside this growing consensus of research institutes, analysts, and security agencies on both sides of the Atlantic, think of climate change as a risk multiplier; as something that will amplify existing social, political, and resource stressors,” said Janani Vivekananda of International Alert, speaking at the Wilson Center on May 10. Vivekananda, a senior climate policy officer with International Alert’s Peacebuilding Program, was joined by co-presenter Jeffrey Stark, the director of research and studies at the Foundation for Environmental Security and Sustainability (FESS), and discussant Cynthia Brady, senior conflict advisor with USAID’s Office of Conflict Management and Mitigation, to discuss the complex connections between climate change, conflict, stability, and governance.
A Multi-Layered Problem
Climate change and stability represent a “double-headed problem,” said Vivekananda. Climate change, while never the only cause of conflict, can increase its risk in certain contexts. At the same time, “states which are affected by conflict will already have weakened social, economic, and political resilience, which will mean that these states and their governments will find it difficult to address the impacts of climate change on the lives of these communities,” she said.
“In fragile states, the particular challenge is adapting the way we respond to climate change, bearing in mind the specific challenges of operating in a fragile context,” said Vivekananda. Ill-informed intervention programs run the risk of doing more harm than good, she said.
For example, Vivekananda said an agrarian village she visited in Nepal was suffering from an acute water shortage and tried adapting by switching from rice to corn, which is a less water-intensive crop. However, this initiative failed because the villagers lacked the necessary technical knowledge and coordination to make their efforts successful in the long term, and in the short term this effort actually further reduced water supplies and exacerbated deforestation.
“Local responses will only be able to go so far without national-level coordination,” Vivekananda said. What is needed is a “harmony” between so-called “top-down” and “bottom-up” initiatives. “Adapting to these challenges means adapting development assistance,” she said.
“What we’re finding is that the qualities that help a community, or a society, or in fact a government be resilient to climate change are in fact very similar qualities to that which makes a community able to deal with conflict issues without resorting to violence,” said Vivekananda.
No Simple, Surgical Solutions
“The impacts of environmental change and management of natural resources are always embedded in a powerful web of social, economic, political, cultural, and historical factors,” said Stark. “We shouldn’t expect simple, surgical solutions to climate change challenges,” he said.
Uganda and Ethiopia, for example, both have rich pastoralist traditions that are threatened by climate change. Increasing temperatures, drought, infrequent but intense rains, hail, and changes in seasonal patterns are threatening pasture lands and livelihoods.
At the same time, pastoralists are confronting the effects of a rapidly growing population, expanding cultivation, forced migration, shrinking traditional grazing lands, anti-pastoralist attitudes, and ethnic tensions. As a result, “any intervention in relation to climate adaptation – whether for water, or food, or alternative livelihoods – has to be fully understood and explicitly acknowledged as mutually beneficial by all sides,” Stark said. “If it is seen in any way to be favoring one group or another it will just cause conflict, so it is a very difficult and delicate situation.”
Yet, the challenges of climate change, said Stark, can be used “as a way to involve people who feel marginalized, empower their participation…and at the same time address some of the drivers of conflict that exist in the country.”
Case Studies: Addressing the “Missing Middle”
When doing climate change work in fragile states, “you have to think about your do-no-harm parameters,” said Brady. “Where are the opportunities to get additional sustainable development benefit and additional stabilization benefit out of reducing climate change vulnerability?”
More in-depth case studies, such as the work funded by USAID and conducted by FESS in Uganda and Ethiopia, are needed to help fill the “missing middle” between broad, international climate change efforts – like those at the United Nations – and the community level, Brady said.
The information generated from these case studies is being eagerly awaited by USAID’s partners in the Departments of State, Defense, and Treasury, said Brady. “We are all hopeful that there will be some really significant common lessons learned, and that at a minimum, we may draw some common understanding about what climate-sensitive parameters in fragile states might mean.”

How does globalization affect natural resource issues such as water on local, national, and international levels? Can our common dependence on these stressed resources be a force for bringing people together rather than dividing us? What lessons can we learn from sharing insights from communities at these very different levels of organization?
Pathways to Peace
In January 2010, the Woodrow Wilson International Center for Scholars and the Fetzer Institute invited 22 scholars and practitioners to a two-day seminar to discuss these questions and the deep connection between caring for the environment and caring for community. Pathways to Peace: Defining Community in the Age of Globalization was the second seminar in a three-year initiative to combine scholarship, public policy, and local practice to articulate and support global conflict transformation and reconciliation in communities throughout the world. Examining the effect of environmental peacebuilding on communities, the discussion explored how governments, NGOs, the private sector, and other interested parties can generate positive outcomes while minimizing negative ones.
Participants from Canada, Ecuador, Ethiopia, Israel, Kenya, Nepal, Switzerland, the Philippines, and the United States brought to the table a wide range of experience and expertise from diverse fields, including peacebuilding, community building, health care, economic development, conflict resolution, and foundation management. By convening leaders in environmental peacebuilding and community building, the Wilson Center and the Fetzer Institute drew on a wide range of experience and perspectives related to environment, conflict, and peacebuilding practice and research. The group used water access and peacebuilding case studies as a means to enter into dialogue about the challenges of global community engagement.
Shared Waters
In preparation for the seminar, geographer and renowned water expert Aaron Wolf of Oregon State University contributed a paper, “The Enlightenment Rift and Peacebuilding: Rationality, Spirituality, and Shared Waters,” in which he laid out the complicated, sometimes conflictual, and often surprisingly collaborative aspects of negotiations over water resources. For Wolf, given water’s life-sustaining quality but limited quantity, it seems intuitive that “water should be the most conflictive of resources.” However, he maintains that “while press reports of international waters often focus on conflict, what has been more encouraging is that, throughout the world, water also induces cooperation, even in particularly hostile basins, and even as disputes rage over other issues…there is a long, and in many ways deeper, history of water-related cooperation.”
On this foundation, Wolf illustrates four stages of water conflict: from adversarial, to reflective, to integrative, to action. Lessons from the “spiritual understanding of water conflict transformation” he says, “offer not only new understanding of current disputes, but also models, tools, and strategies for more effective water conflict management and transformation.”
Seminar participants used Wolf’s paper as a starting point from which to write short papers based on their own expertise and experience. From Kenya to Nepal to Harlem, participants shared their perspectives on the challenges and promises of environmental issues, community building and organizing, and peacebuilding.
This report, Our Shared Future: Environmental Pathways to Peace, draws from the rich dialogue of the seminar and seminar papers to share the broad range of experience and the insight of the participants. Integrating Development: A Livelihood Approach to Population, Health, and Environment Programs
Rural communities in developing countries understand that high population growth rates, poor health, and environmental degradation are connected, said Population Action International’s Roger-Mark De Souza at a recent Wilson Center event. An integrated approach to development – one that combines population, health, and environment (PHE) programs – is a “cost-effective intervention that we can do very easily, that responds to community needs, that will have a huge impact that’s felt within a short period of time,” said De Souza. “This is how we live our lives, this makes sense to us – it’s completely logical,” community participants in PHE projects told him.

March 9, 2011 // By Schuyler Null“Countries that have a high proportion of young people are typically more prone to political violence,” said demographer-in-residence at the Stimson Center, Richard Cincotta, in this interview with ECSP. “That is, not necessarily international war [but] internal conflict, which may take different forms,” including civil and ethnic strife, domestic terrorism, and violent political demonstrations.MORE
The role of unemployed and angry youths in the recent unrest that has swept the Middle East has received a great deal of coverage, but though the region in general is very young, some countries are more so than others.
Tunisia (median age of 29) is actually well into its demographic transition, where fertility declines towards replacement level. “Fertility – the number of children women have in their lifetime – is now lower than it is in the United States,” said Cincotta. As a result, Tunisia’s prospects for achieving a stable, liberal democracy – based on the historical relationship between age structure and political freedoms (see Cincotta’s full post on Tunisia and the two follow-ups for a more complete treatment of that relationship) – are about even.
In contrast, Egypt’s age structure remains young (median age of 24) and Yemen’s (median age of 17) is extremely young. “Those difference are very stark,” said Cincotta, and they play out in the risk of political violence: Tunisia is less likely to experience continuing political violence; Egypt, more so; and Yemen, even more likely.
The relationships between age structure and political violence and the emergence of democratic institutions can be useful in other conflict-prone regions as well. “Afghanistan, Pakistan, Iraq, the Palestinian Authority, most of the central band of sub-Saharan Africa – from Nigeria to the Congo, to Kenya and Ethiopia – we know that these countries are volatile, we’re not always sure why,” said Cincotta. But “age structure gives you a clue, because it tells you something about a lot of barriers that are important to development.”
