PRB Maps the PHE World
›The Population Reference Bureau (PRB) has created an interactive Google map that highlights population, health, and environmental programs in Africa, Asia, and Latin America. The Population, Health, and Environment (PHE) Map shows the locations of projects that combine improved health services, sustainable natural resource management, and ecosystem conservation.
According to PRB’s Maura Graff and Jason Bremner, “the map aims to give viewers both a sense of the scale of current PHE integration efforts, and specific information about individual organizations, projects, and their location.”
Clicking on a marker will reveal the title of a PHE program, a short description of its goals and outcomes, a link to its website, and the occasional picture of the project in action. A list on the left alphabetically catalogs all the projects based on region.
The map also provides the opportunity for those working on PHE projects in the field to find nearby projects in order to share experiences. For example, in Ethiopia, Population Media Center and DSW’s Youth to Youth could share lessons learned about disseminating information on the reduction of female genital mutilation, the importance of providing family planning services to young married couples, and the linkage between reduced family size and enhanced stewardship of social and natural resources.
Communities that understand how population, health, and environment are intertwined are more engaged and energized to make a difference. In a video interview with ECSP, Roger-Mark De Souza, director of foundation and corporate relations at the Sierra Club, explains that PHE projects are so effective because solutions to a community’s liked problems demand this logical integration. For example, while learning about the health benefits of birth spacing, father and mothers also learn how their family planning decisions can improve the economic and environmental prospects of the community.
Just like PHE projects themselves, the map is a work in progress; PRB is seeking to add PHE projects that are still active or ended after 2005. Please contact Maura Graff if you would like to add a project.

A Return to Rural Unrest in Nepal?
›July 27, 2010 // By Russell SticklorIn the four years since the end of Nepal’s civil war, political progress in creating a multi-party unity government in Kathmandu has moved in fits and starts. While the effort to bring the Maoists into the fold has made some headway since 2006, continuing environmental and economic troubles in the Nepalese countryside threaten to undermine these tentative steps.
In recent months, a new threat to political stability has emerged: the Sapta Kosi Multipurpose Project, a massive, India-backed hydropower scheme in eastern Nepal currently in the early stages of development. Once operational, the controversial dam—slated to reach a height of nearly 270 meters, making it one of the tallest dams in the world—is projected to generate 3,300 megawatts of electricity.
A proposed barrage and series of canals round out the project, enabling new irrigation and flood-control infrastructure in both eastern Nepal and the Indian state of Bihar, immediately to the south. But the potential environmental impacts of the mega-project have already sparked significant backlash among some Maoist-linked ethnic groups in the region, where the reach and influence of Nepal’s fledgling unity government is tenuous at best.
“Strong” Protests Threatened Over India-Backed Mega-Dam
In June, a network of 15 groups sympathetic to the Nepalese government’s Maoist wing warned of “strong” protests if survey work on the dam continued and “the voice of the indigenous people was not heard.” A memo released by the group dismissed the Sapta Kosi project as “anti-people.”
Specific criticisms of the project have ranged from safety concerns (the dam would be built in a seismically active region) to population displacement. Maoist leaders in the region have alleged that many villages—as well as important local religious sites and valuable agricultural land—could be flooded if the project goes forward. Other Maoists say the project should be delayed until Nepal is reorganized as a federal republic, at which point the states directly impacted by Sapta Kosi could be given greater control over the project.
Meanwhile, some objections to the project have targeted Nepal’s partnership with India. According to ShanghaiNews.net, members of the Maoist opposition have insinuated that hydropower from Sapta Kosi will not be consumed domestically, but rather exported to meet the needs of energy-hungry India.
A number of prominent Nepalese and Indian environmental activists have also spoken against the project, including Medha Patkar, a well-known activist who has played a major role in many past Indian anti-dam protests. Patkar warns the project will not mitigate but instead worsen seasonal flooding, calling plans for the joint India-Nepalese dam project “inauspicious from [an] environmental, cultural and religious point of view,” according to the Water & Energy Users’ Federation-Nepal.
As Nepal Pledges Security for Dam Project, India Pushes Forward
In the past, threats against the Sapta Kosi project have caused surveillance work in the area to be suspended repeatedly. But after the latest round of warnings, the Nepalese government adopted a different tactic, pledging heightened security in the region to ensure the safety of Indian officials doing fieldwork.
In doing so, Nepal’s coalition government is throwing its limited weight around, and—to a degree—staking its reputation on its ability to prevent an outbreak of violence. Historically, Nepal’s government has been largely bypassed or ignored in matters of hydroelectric development. As Nepal Water Conservation Foundation Director Dipak Gyawali told International Rivers in a June 2010 interview:The main players are private investors, with state entities and civil society unable to stand up to them….In Nepal, we just saw local politicians burn down the office of an international hydropower company even after the project was sanctioned by their leaders in the central government.
Gyawali added that during the Nepalese civil war (1996-2006), private developers were able to build “small hydropower projects even while a Maoist insurgency was raging because they did not ride roughshod over local concerns.” Regarding Sapta Kosi, Gyawali said the government should adopt a similar approach, and “start listening to the marginalized voices.” Otherwise, he warned, the Indian-Nepalese team spearheading the project “will be faced with delays, impasse, and intractable political problems,” including the potential for Maoist violence in the region. (As noted earlier this month in New Security Beat, the Indian government has also struggled with Maoist-linked violence in recent years, as New Delhi struggles to pacify a Naxalite insurgency in eastern and central India.)
Rural Nepal’s Troubles Far Bigger Than Sapta Kosi
Maoists may be wielding Sapta Kosi as a weapon to gain political leverage both in the countryside and Kathmandu, but the proposed dam is far from the only environmental issue impacting rural lives and threatening to undermine support for the central government.
In a country where firewood still accounts for 87 percent of annual domestic energy production, deforestation has been hugely problematic across rural Nepal. As of 2010, less than 30 percent of the country’s original forest-cover now remains. The rapid removal of forest cover has reduced soil quality, exacerbated seasonal flooding, and caused degraded water quality due to high sedimentation levels.
Further, as the country’s population grows at an annual rate of 2 percent, low soil productivity and unsustainable farming practices have turned Nepal’s effort to feed itself into a constant uphill struggle. According to the World Bank, the country sports one of the world’s highest ratios of population to available arable land, paving the way for potential further food shortages.
Sustainable energy development in Nepal perhaps represents one way of slowly restoring environmental health to the country. By investing in a more reliable national power grid, the central government could reduce rural dependence on firewood for fuel, allowing the country’s forests, soil, and waters to recover even as population increases. Further, hydroelectric projects like Sapta Kosi—implemented with greater involvement from local communities—could play an important role in moving the country forward. With an estimated untapped hydroelectric potential of 43,000 megawatts, Nepal could not only meet its own energy needs by developing its waterways, but profit from hydroelectric energy exports as well.
On the other hand, the Nepalese government could—at its own peril—continue to overlook rural populations’ grievances, and the environmental degradation unfolding outside Kathmandu. If left unchecked, however, these conditions could once again make the Maoist insurgency an appealing movement, potentially reviving grassroots support for anti-government extremism.

Stephanie Hanson Reports on PHE in Agricultural Development and Rwanda’s ‘One Acre Fund’
›Driving from Kigali into rural Rwanda, the hills that flank either side of the paved road are covered with bananas, maize, coffee, and beans under cultivation. Most Rwandans are farmers, using any bit of available land to feed their families and generate income. In this country—the most densely populated in Africa—little arable land is left untended.
My organization, One Acre Fund, offers loans and education to smallholder farmers in Kenya and Rwanda. We work with 18,000 farmers in three districts in the southwestern and western part of Rwanda, where we are know as Tubura, which means “multiply” in Kinyarwanda.
Though One Acre Fund is not a traditional population, health, and environment (PHE) project, agricultural development work inherently is PHE work, particularly in Rwanda, which faces significant population and environment challenges.
Our farmers have small plots of land because Rwanda’s population density is so high—375 people per square kilometer, higher than Japan—leaving only .13 hectares of arable land per person. They struggle to grow enough food because it’s difficult to support a big family on a small piece of land, especially without access to high-quality seed and fertilizer.
When farmers don’t grow enough to ensure basic food security for their families, their children are malnourished, which makes them more susceptible to illness.
Finally, agriculture both depends on and affects the environment. Farmers need favorable growing conditions—good soil and adequate rainfall—for a good harvest. Sustainable agriculture practices, such as composting and preventing soil erosion, ensure the environment remains healthy to support future farming.One Acre Fund is acutely aware of the challenges that our farmers face due to high population density, food insecurity, and environmental degradation. We offer a service model that addresses all the needs of a smallholder farmer: financing, farm inputs, education, and market access.
When a farmer enrolls with One Acre Fund in Rwanda, she joins as part of a group of 6-15 farmers. She receives an in-kind loan of seed and fertilizer, which is guaranteed by her group members. One Acre Fund delivers this seed and fertilizer to a market point within two kilometers of where she lives. A field officer provides in-field training on composting, techniques to prevent soil erosion, land preparation, planting, fertilizer application, and weeding.
Over the course of the season, the field officer monitors the farmer’s fields. At the end of the season, he trains her on how to harvest and store her crop. One Acre Fund also offers a harvest buyback program that farmers can choose to participate in.
On average, One Acre Fund farmers double their farm income per acre in one growing season. Ninety-eight percent of our farmers repay their loans, which are due several weeks after harvest.
With their increased harvests, One Acre Fund farmers are able to feed their children, which reduces malnutrition. Anecdotally, we also know that One Acre Fund children experience less illness; this year, we are working to incorporate health indicators into our monitoring and evaluation work.
At a harvest buyback last month, I met many farmers who had benefited from One Acre Fund’s services. One woman, Tamar, had sold 400 kilograms (880 pounds) of beans at the previous season’s buyback, which earned her roughly 132,000 Rwandan francs ($235 USD). She told me that she was using the money to build a bigger home for the six of her ten children who lived at home.
However, Tamar really wanted to buy a cow, but she knew that she would not earn enough money this year to afford one. With so many children, she struggled to earn enough money to invest in something that might generate additional income for her and her family.
Another woman, Medeatrice, had also made $235 USD from the sale of her beans. With that income, she had opened a small shop with her husband in a nearby market. Unusually for Rwanda, where the average woman has 5.5 children, Medeatrice only had one, a three-year old boy named Prince. I asked her if she planned to have more children.
“I only want one more child,” she told me. “If I only have two children, it is easy to educate and to take care of them.”
The Rwandan government has invested in educating its population on family planning, but it will take time for birth rates to drop. For now, families with five, six, or nine children are not uncommon.
However, research shows that when women have increased access to economic opportunities, birth rates drop. One Acre Fund is focused on helping Rwanda’s families increase their harvests so that they not only have enough to eat, but they can start investing in their futures.

Women Deliver in the Climate Change Debate
›One of the hottest topics at the “Women Deliver” conference earlier this month—where panels ran the gamut from HIV prevention and family planning to gender-based violence and maternal health—was the intersection of women’s reproductive health, global population growth, and climate change.
As panelists at three of the conference’s climate-focused events noted, women in poor, rural areas are especially vulnerable to the impacts of climate change. In many developing countries across Africa, Asia, and Latin America, women take on much of the burden of farming, gathering fuel, and supplying fresh water for their communities. As a result, they bear the brunt of hardships when climate change alters seasonal precipitation patterns, or increases scarcity of key natural resources.
In addition, “the more assets, the less vulnerable one person is,” said Lorena Aguilar of the International Union for Conservation of Nature. “Worldwide, compared to men, women tend to have more limited access to resources that would enhance their capacity to adapt to climate change—including land, credit, agricultural inputs, decision-making bodies, technology, and training services.”
Women’s hardship in the face of climate change can also have a negative effect on reproductive health. Aguilar remarked that during the dry season in parts of rural India and Africa, 30 percent or more of women’s daily caloric intake is spent on fetching water alone. The enormous physical strain placed on women’s bodies because of those tasks has resulted in higher miscarriage rates among those populations, she noted.
Educating Girls to Protect the Environment
Organizations like the United Nations and the Global Gender and Climate Alliance have been working in recent years to bridge the gap between women’s rights and climate change, and reframe climate change in terms of human development. But to date, women’s struggles with climate change have not translated into meaningful economic, educational, or healthcare support at the local government level, with women’s welfare “at the very bottom of the priority list” for most developing countries, according to Nickie Imanguli with Advocates for Youth.
The unmet need for family planning tools and services is perhaps the movement’s principal challenge going forward. With an estimated 200 million women having an unmet need for family planning, unintended pregnancies could be exacerbating environmental problems such as depletion of forests, water, and other finite resources. But most panelists expressed optimism that the growing recognition of a connection between climate change and women’s reproductive health might lead to a boost in funding for family planning initiatives in underserved areas of the world.
Speakers at Women Deliver emphasized that reproductive health can be bolstered by improving educational opportunities for girls in poor rural areas. Joy Nayiga with Uganda’s Ministry of Finance Planning Economic Development noted that “girls are more likely than boys to drop out of school to help their mothers gather fuel, wood, and water.” This trend, she said, robs females of an opportunity for educational advancement, and heightens the likelihood they will end up starting families of their own while very young.
Nayiga and other panelists asserted that empowering females through education leads them to take greater control over their own sexual health, making it easier for them to start their families later in life, or perhaps have a smaller number of children.
Encouraging women to take a more active role in family planning in this regard serves as “a win-win situation for women, their communities, and the nations of the world,” by “bending down the overall trajectory of population growth,” asserted the Worldwatch Institute’s Robert Engelman.
Some speakers also argued that enabling women to delay motherhood if they want could yield direct environmental benefits for nations of the Global South that are struggling to adapt to climate change. Since women are often responsible for overseeing agriculture and forest resource management practices in their communities, they help create localized carbon sinks across the developing world.
“Women pull carbon out of the atmosphere and bury it, in farm soils, in trees that they grow,” noted the Worldwatch Institute’s Engelman, who even suggested women’s aggregate impact removing carbon could be more effective than cap-and-trade plans.
Moving Slowly From Talk to Action
Given both their vulnerability to the effects of climate change—and their potential to help offset those same impacts—“women are critical stakeholders in climate change moving forward,” said Population Action International’s Kathleen Mogelgaard. So far, however, while there may be growing discussion about giving women a more prominent seat at the table when developing climate change adaptation and mitigation plans, that has not yet happened.
“We’re not seeing big government investment in empowering women on the issue of climate change,” remarked Leo Bryant, with Marie Stopes International, a U.K.-based NGO specializing in sexual and reproductive health. Instead, Bryant said, it has been NGOs that have been doing much of the heavy lifting of bringing women into the conversation.
But many panelists felt that, in time, governments will recognize it is in their enlightened self-interest to link issues of gender rights and climate change. “By upholding women’s rights,” concluded the IUCN’s Lorena Aguilar, “we are in fact making one of the most crucial preparations associated to climate change that any society can make.”

The Contradictions That Define China
›As a China follower who has visited the country numerous times over the past 40 years, I have an enduring love affair with the “old” China, which prided itself on balancing the harmony of nature with its decision-making. It is tragically ironic that despite this impressive historical and cultural backdrop, current choices have pushed the country’s harmony with nature beyond the tipping point.
The atmosphere in Chengdu, the capital of Sichuan Province in central China, tells a cautionary tale of China’s current emphasis on economic growth at the expense of its citizens’ health. With lower annual sunshine totals than London, Chengdu’s perpetually grey, cloudy horizon graphically illustrates the massive industrial waste and coal-fired pollution that plague this booming city.
In April, global health experts convened in Chengdu at the 5th International Academic Conference on Environmental and Occupational Medicine, which was co-sponsored by the U.S. Centers for Disease Control and Prevention and Chinese CDC offices, to report on the impact of climate change on public health.
Because of its geography, the mountain-encircled basin in which Chengdu sits is the natural point of release for daily deposits of air pollutants and dust blown in from India and other countries to the west. A rapid series of satellite views presented by John Petterson, director of the Sequoia Foundation, brought gasps from conference attendees, as these accumulated industrial wastes were shown concentrating against the massive southern Tibetan mountain range, then turning grey and black as they moved east, before being deposited on Sichuan’s vulnerable (and already heavily polluted) basin.
Environmental risk factors, especially air and water pollution, are a major–and worsening–source of death and illness in China. Air quality in China’s cities is among the worst in the world, and industrial water pollution is a widespread health hazard.
And although air pollution is clearly a complex global problem, individual nation-states continue to approach this dilemma as though it lies between its borders alone.
Luckily, this conference was prefaced by a number of timely scientific articles on China in the Lancet. The predominantly Chinese authors clearly take the critical risks that we expect from the Lancet, which is a defender of accountability and transparency in global health. This issue brings shame to other publications who still consider their “place” to be protecting the conventional politico-economic emphasis on decision-making inherent to most industrialized countries.
At the conference, public health and climate experts openly voiced their frustration at the blatant privileging of economic priorities over the health of unknowing populations. China–the fastest growing economy in world history, with unprecedented growth driven by external “demand” and precipitous acceleration of domestic consumption–has become the poster-child for global ignorance. However, the West is an equal and essential partner, having moved their manufacturing to China for cheap labor, lax environmental regulations, and higher profits.
Dr. Mark Keim, senior advisor for the CDC’s National Center for Environmental Health, presented irrefutable evidence that root structure erosion from advancing saltwater has led to starvation in Polynesia, the first recorded evidence-based outcome measure of climate change’s public health effects.
At Mark’s request, I presented evidence on the worsening health impacts of rapid urbanization, specifically within urban enclaves that expand due to dense population growth before protective public health infrastructures and systems are in place (forthcoming in Prehospital and Disaster Medicine). Currently, the megacities in Asia and Africa–where sanitation is ignored and infectious disease is prevalent–now have the highest infant and under-age five mortality rates in the world.
Today, we face the largest gap in health indices between the haves and have-nots since the alarming days before the Alma-Ata Declaration. This new health data was an uncomfortable surprise to many conference attendees, most of whom were experts in the physical and environmental sciences.
We have become too “vertical” in our research over the past 50 years, and thus we fail to recognize that solutions to problems facing the global community must be trans-disciplinary and multi-sectoral, and serve multiple ministries and decision-makers.
If not undone by human decisions, global climate change and climate-warming greenhouse gases will rapidly intensify. This effort requires the best collaboration between science and the humanities, as well as the harmonious lessons of the “old” China.

Population and Environmental Challenges in Rwanda
›“Population, Health and Human Settlements” is the second chapter in the Rwandan government’s Rwanda State of Environment and Outlook. The chapter highlights the Rwandan government’s recognition of the interconnections between population, health, and environment, noting that population “can influence the state of the environment” and pose strains “on available public infrastructure, limited land, and natural resources.” The chapter examines Rwanda’s population growth and distribution, the state of “environmental health” in rural and urban areas, and health indicators relating to child and maternal health and HIV/AIDS. It goes on to describe government strategies to “improve settlements and human welfare.” “As population pressure is one of the key drivers of environmental degradation and poverty,” the chapter’s authors write, “the implementation of the population policy, especially aspects that address high fertility rates, gender and reproductive health, migration and human settlements” is increasingly important.The Des Moines Register‘s “Renewal in Rwanda” site hosts a series of articles by IRP Fellow and former Wilson Center Public Policy Scholar Perry Beeman “examining Rwanda’s efforts to build an eco-friendly economy.” Accompanied with interactive maps, photos, and videos, the materials highlight government efforts, share the country’s successes, and describe the vast challenges that lay ahead.
“Renewal in Rwanda” is particularly focused on Gishwati Forest, an area Beeman visited while in Rwanda, and the impacts of its ongoing conservation program on local communities. “Gishwati Area Conservation Program has as much to do with saving the lives of villagers—by sparing them from deadly mudslides and providing them jobs—as it does restoring a once-mighty forest,” writes Beeman in the article “Fighting for an African Forest.” Beeman also calls attention to the program’s more controversial aspects, noting that reforestation efforts would require relocating an estimated 5,000 families. -
City Living: World Health Day 2010 Focuses on Urban Health
›April 7, 2010 // By Julien KatchinoffCelebrating World Health Day, the World Health Organization, with its partners around the globe, today launched an initiative for healthly lives in urban settings, through the theme “1000 Cities, 1000 Lives.” “We are at a critical turning point in history where we can make a difference,” said Dr Ala Alwan, assistant director-general for noncommunicable diseases and mental health.
Since the first World Health Day 60 years ago, the world has seen a dramatic rural exodus. Today, more people live in urban areas than anywhere else. Providing healthy livelihoods for the urban poor is a challenge, as poverty-stricken urban centers face a number of health obstacles, from high child mortality rates, environmental pollution, and widespread disease, to a lack of access to basic water, sanitation, and health care.
“In general, urban populations are better off than their rural counterparts,” said WHO Director-General Margaret Chan. “They tend to have greater access to social and health services and their life expectancy is longer. But cities can also concentrate threats to health such as inadequate sanitation and refuse collection, pollution, road traffic accidents, outbreaks of infectious diseases and also unhealthy lifestyles.”
The 1000 Cities campaign hopes to encourage all cities to promote healthy activities during the week following World Health Day (4/7-4/11). Through a new website, the WHO is collecting profiles and pictures in an easy-to-navigate map. Notable activities include HIV/AIDS-awareness flashmobs, skateboarding and cycling competitions, car-free days, outdoor sports events, and dance performances, in cities as diverse as Mandalgobi, Mongolia; Bangalore, India; and Luanda, Angola.
The Wilson Center’s Environmental Change and Security Program and its Comparative Urban Studies Program have collaborated on urbanization events and publications that examine the strong and critical connections between healthy urban populations and environmental sustainability.
With proper attention paid to delivering population, health, and environmental services in our urban centers, it may be possible to leverage the benefits of higher urban densities—such as lower aid dispersion costs, communication access, infrastructure services, and fertile environments for ideas and productivity—to ensure urban sustainability for the future. -
Population, Health, and Environment
›The WWF and Equilibrium Research released a report on the interplay between the environment and human health. Vital Sites: The Contribution of Protected Areas to Human Health documents the environmental-human health connection, provides case studies from both the developed and developing worlds, and offers recommendations to enhance the health outcomes that can be gained from environmental good governance. “[P]rotected areas are not a luxury but are key sites to protect not only biodiversity, but also ecosystem services and our wider well-being,” the World Bank’s Kathy MacKinnon writes in the foreword.“Family Planning and the Environment: Connected Through Human and Community Well-Being,” part of PATH‘s Outlook series, details the importance of family planning-environmental projects to communities living in remote and ecologically vulnerable areas. Designed for practitioners, the article aims to promote cross-discipline dialogue and offers case studies from the Philippines and Uganda. The article concludes that “more collaborative family planning and environmental efforts aimed at reducing inequities would better ensure sustainable community development as well as the right of individuals to achieve what they value.”
