“There Is No Choice:” Climate, Health, Water, Food Security Must Be Integrated, Say Experts
›August 9, 2010 // By Russell SticklorBureaucratic stovepipes plague international development efforts, and aid for pressing environmental and human security concerns—such as climate change, food shortages, fresh water access, and global health threats—rarely matches the reality on the ground in the developing world, where such health and environmental problems are fundamentally interconnected.
Instead, development efforts in the field—whether spearheaded by multilaterals, bilaterals, or NGOs—are commonly devoted to single sectors: e.g., the prevention and treatment of a single disease; the implementation of irrigation infrastructure in a specific area; or the introduction of a new crop in a certain region. The reasons for such a narrow focus can come from multiple sources: finite resources, narrowly constructed funding streams, emphasis on simple and discrete indicators of success, and institutional and professional development penalties for those who conduct integrated work. But some experts argue that integrating problem-solving initiatives across categories would not only improve the efficacy of development efforts, but also better improve lives in target communities.
As part of the USAID Knowledge Management Center‘s 2010 Summer Seminar Series, a recent National Press Club panel on integration featured a frank discussion of both the opportunities and challenges inherent in breaking down barriers within and between development agencies. Panelists from the World Bank’s Environment Department, the White House Council on Environmental Quality, and the Woodrow Wilson Center’s Environment Change and Security Program weighed in on the prospects for cross-sectoral integration.
Addressing the impacts of a global problem like climate change “requires multilevel approaches,” and necessitates that we “think multisectorally along the lines of agriculture, water, transportation, energy, [and] security,” said Loren Labovitch of the White House Council on Environmental Quality. The four topics under discussion—climate change, food security, water, and health—are all Obama administration priorities, as reflected by dedicated programs and special initiatives. Finding ways to practically integrate these interrelated challenges (through efforts like the Feed the Future Initiative or the Global Health Initiative) is getting more attention from policy analysts and policymakers with each passing year.
Integration in Practice: Success Stories
While there may be an emerging willingness to discuss and even experiment with holistic programming, what does it actually look in practice? Panelist Geoff Dabelko, director of the Woodrow Wilson Center’s Environmental Change and Security Program, singled out integrated development programs in the Philippines, the Democratic Republic of Congo, and Asia as examples.
Philippines: The PATH Foundation Philippines’ Integrated Population and Coastal Resource Management (IPOPCORM) initiative uses an integrated approach to address health and environmental concerns in coastal communities. Their “basket of services” includes establishing a locally managed protected marine sanctuary to allow local fish stocks to recover, promoting alternative economic livelihoods outside of the fishing industry, and improving access to local health services and commodities, said Dabelko. To date, IPOPCORM has yielded several notable improvements, among them reduced program costs and improved health and environmental outcomes as compared to side-by-side single sector interventions. A forthcoming peer-reviewed article will appear in Environmental Conservation, and will detail the controlled comparison study of the IPOPCORM project.
Democratic Republic of Congo: Mercy Corps has also successfully pursued cross-sectoral programming as part of a larger effort to be more holistic in its humanitarian and development responses. In war-torn eastern DRC, Mercy Corps brought practitioners with expertise in natural resource management into the fold of what has historically been an emergency relief mission. In particular, the Mercy Corps mission has fused humanitarian assistance with longer-term development efforts such as enhanced environmental stewardship. For example, promoting the use of fuel-efficient cookstoves eases pressure on local forest resources by lowering the need for firewood, and improves respiratory health by lowering air pollution. The project scaled up the effort through resources from further integration, with carbon credits from avoided emissions being sold through a local broker to the European cap and trade market. These resources in turn helped finance more cook stoves, which now total 20,000 for this project.
“The lesson is we have no excuse for not doing this anywhere in the world and saying some place is too unstable,” Dabelko said. “If we can do it [integrated projects] in eastern DRC, we should be able to do it anywhere.”
Asia: Tackling programmatic integration starts with better understanding the interconnections between environmental and health challenges. Dabelko cited a recent effort of the environment and natural resources team within USAID’s Asia Bureau as an example of breaking out of narrow bureaucratic stovepipes.
USAID staff recognized that a wide set of climate, energy, economic, governance, and conflict issues affected their core biodiversity and water portfolios, even if they did not have the time, expertise, or resources to investigate those issues in detail. Trends that appeared to be in the periphery were not viewed as peripheral to planning and designing programs for long-term success.
Working with the Woodrow Wilson Center, the USAID Asia Regional Bureau engaged experts on a diverse set of topics normally considered outside their portfolios. The resulting workshop series and report led to a deeper understanding of the possible impacts of increased Himalayan glacier melt and Chinese hydropower plans on food security and biodiversity programs in the lower reaches of the Mekong River. Bringing analysis from these topically and geographically remote areas into local-level development planning is a process that will require a similar willingness to go outside the typical bounds of one’s brief.
More Integration Ahead?
These case studies provide a glimpse of what integrated programming can look like on the ground. Still, significant hurdles remain standing in the way of regular and effective integration. Cross-sectoral programming demands that old problems be addressed in innovative and perhaps unfamiliar ways, requiring the addition of new capacity in development organizations and better coordination within and between agencies. That can be a complicated process, noted Dabelko, since efforts to pursue greater programming integration can be “hamstrung by earmarks and line items.”
Integration can also prove tricky because it requires a greater willingness to accept multiple indicators of success unfolding over different time frames—health gains may occur quickly, for example, while progress on environmental conservation may unfold less speedily. This means existing programs might need to be reshaped and reoriented to accommodate these divergent time frames, which could prove somewhat difficult. “Integration can be a challenge, both from a programming perspective and from an organizational perspective,” acknowledged moderator Tegan Blaine, climate change advisor for USAID’s Africa Bureau.
Further, the temptation remains strong among appropriators and implementers alike to maintain control over authority and resources in their traditional portfolios. Getting long-time practitioners in particular issue areas to willingly cede some of their turf in the pursuit of greater integration has historically been the “real world” that stands in the way of such integrated work.
But, as shown by the standing-room-only crowd at the seminar, momentum is slowly starting to build in pursuit of breaking down old programming walls and finding new approaches to addressing emerging challenges in human and environmental security.
“There is no choice” but to fuse development agendas with climate change adaptation efforts, asserted Warren Evans, director of the World Bank’s Environment Department. “It can’t be a parallel process anymore.”

Population Reference Bureau Releases New Projections
Seven Billion and Counting
›August 6, 2010 // By Russell SticklorThe Population Reference Bureau recently released its annual World Population Data Sheet, and lo and behold, the world is getting more crowded. By next year, the global total is expected to top seven billion, as we march toward upwards of nine billion by mid-century.
Much of that growth, of course, will be unfolding across Asia — China and India already account for more than 35 percent of the world’s population, and the two countries will continue to drive global demographic change. But the PRB report emphasized that Africa will also contribute significantly to world’s shifting demographics in the coming decades. By 2050, the continent’s population is slated to double, reaching the neighborhood of two billion. In that time span, the Democratic Republic of Congo and Ethiopia are expected to knock Russia and Japan off the list of the world’s top 10 most populous nations.
The Young Continent
Africa’s population growth through 2050 could actually be even greater than PRB predicts, since current projections assume that total fertility rate in sub-Saharan Africa will drop from 5.2 to 2.5 in that time span. The assumption is that as the region’s countries travel farther down the path to economic modernization, access to family planning services will become more widespread, resulting in smaller average family sizes—a trend well-documented in other parts of the developing world.
A slower than expected decrease in total fertility rate (TFR) across sub-Saharan Africa over the next 40 years could render current predictions inaccurate, however. While TFR has dropped significantly over the past 50 years in countries like Ghana (where it is now 4.0), just 17 percent of married women in sub-Saharan African utilize a modern type of family planning. That is one of the reasons Africa as a whole maintains a high average TFR of 4.7 children per woman, and why countries like Niger and Uganda sport some of the highest TFRs in the world (at 7.4 and 6.5, respectively).
According to the 2010 PRB Data Sheet, Africa is also notable for its demographic youth bulge. Of the world’s 10 countries with the highest percentage of their populations aged 15 or younger, nine of them—Niger, Uganda, Burkina Faso, the DRC, Zambia, Malawi, Chad, Somalia, and Tanzania—are on the continent. The question facing these countries is whether their young populations will yield a “demographic dividend” during the coming decades, whereby the large youth bulges translate into a sizeable and productive workforce. Harnessing the economic potential of the younger generation could help accelerate the development process in many of the region’s nations, but it will require significant investment in health and education infrastructure—funding that may prove hard to come by for many cash-strapped governments.
Persistent Divides
Other findings featured in the report involved sanitation and wastewater treatment, where some headway is being made at the global level. The PRB Data Sheet reveals that 43 percent of urban populations in sub-Saharan Africa (and 24 percent of the region’s rural populations) now have access to improved sanitation, while 86 percent of urban populations (and 55 percent of rural populations) have better sanitation services in Latin America and the Caribbean. The figures highlight progress, but reveal the significant divide that persists between urban and rural areas in terms of access to quality sanitation. According to the PRB, some 2.7 billion people—40 percent of the world’s total—still do not enjoy adequate sanitation facilities, and most live in rural areas of the developing world.
Finally, an interesting trend that will impact developed and developing nations alike involves elderly support ratios, or the number of active workforce members between the ages of 15 and 64 available to support a nation’s non-working, 65-and-older population. While rapidly growing countries in the developing world will continue to enjoy high elderly support ratios, a number of countries across the developed world will face a potential financial crisis, as comparatively low support ratios combine with rapidly aging populations to strain available financial resources earmarked for covering pension payments and healthcare infrastructure costs. To make up for the shortfall over the coming decades, the PRB report predicts that some developed nations may be inclined to further throw open the doors to immigration to bolster workforce ranks—a step recommended by Jack Goldstone in ECSP Report 13.

Boosting the U.S. Role in the Global Health Arena
›A new video from the Commission on Smart Global Health Policy, which was established by the Center for Strategic & International Studies, reviews the commission’s progress towards its goal of encouraging the U.S. government to embrace global health as a pillar of U.S. foreign policy.
The video reviews the recommendations from the commission’s March 2010 report, A Healthier, Safer and More Prosperous World: 1) Maintain robust U.S. support for the fight against HIV/AIDS, malaria, and tuberculosis; 2) Prioritize maternal and child health, especially in sub-Saharan Africa and South Asia; 3) Help other nations improve their capacity to prevent and respond to outbreaks of contagious disease; 4) Expand U.S. capacity to fund future global health initiatives by securing long-term investments for such efforts; and 5) Step up U.S. funding for multilaterals engaged in the global health field, including the World Health Organization, Global Fund, UNICEF, the World Bank, and the GAVI Alliance.
In the months ahead, commission members will be participating in public forums throughout the United States to discuss and promote the recommendations included in the report, before gathering in January to review the Obama administration’s progress on global health as the administration begins its third year. To date, the centerpiece of the administration’s health outreach efforts has been the six-year, $63 billion Global Health Initiative, designed to promote an enhanced U.S. role in addressing public health issues overseas.
The CSIS Global Health Policy Center will also be launching a year-long debate series called “Fault Lines in Global Health,” focusing on controversial topics in the global health field. The series’ kick-off event will center on U.S. AIDS funding, and is scheduled for Friday, August 6, 2010, from 9:30-11:00 a.m. -
A Return to Rural Unrest in Nepal?
›July 27, 2010 // By Russell SticklorIn the four years since the end of Nepal’s civil war, political progress in creating a multi-party unity government in Kathmandu has moved in fits and starts. While the effort to bring the Maoists into the fold has made some headway since 2006, continuing environmental and economic troubles in the Nepalese countryside threaten to undermine these tentative steps.
In recent months, a new threat to political stability has emerged: the Sapta Kosi Multipurpose Project, a massive, India-backed hydropower scheme in eastern Nepal currently in the early stages of development. Once operational, the controversial dam—slated to reach a height of nearly 270 meters, making it one of the tallest dams in the world—is projected to generate 3,300 megawatts of electricity.
A proposed barrage and series of canals round out the project, enabling new irrigation and flood-control infrastructure in both eastern Nepal and the Indian state of Bihar, immediately to the south. But the potential environmental impacts of the mega-project have already sparked significant backlash among some Maoist-linked ethnic groups in the region, where the reach and influence of Nepal’s fledgling unity government is tenuous at best.
“Strong” Protests Threatened Over India-Backed Mega-Dam
In June, a network of 15 groups sympathetic to the Nepalese government’s Maoist wing warned of “strong” protests if survey work on the dam continued and “the voice of the indigenous people was not heard.” A memo released by the group dismissed the Sapta Kosi project as “anti-people.”
Specific criticisms of the project have ranged from safety concerns (the dam would be built in a seismically active region) to population displacement. Maoist leaders in the region have alleged that many villages—as well as important local religious sites and valuable agricultural land—could be flooded if the project goes forward. Other Maoists say the project should be delayed until Nepal is reorganized as a federal republic, at which point the states directly impacted by Sapta Kosi could be given greater control over the project.
Meanwhile, some objections to the project have targeted Nepal’s partnership with India. According to ShanghaiNews.net, members of the Maoist opposition have insinuated that hydropower from Sapta Kosi will not be consumed domestically, but rather exported to meet the needs of energy-hungry India.
A number of prominent Nepalese and Indian environmental activists have also spoken against the project, including Medha Patkar, a well-known activist who has played a major role in many past Indian anti-dam protests. Patkar warns the project will not mitigate but instead worsen seasonal flooding, calling plans for the joint India-Nepalese dam project “inauspicious from [an] environmental, cultural and religious point of view,” according to the Water & Energy Users’ Federation-Nepal.
As Nepal Pledges Security for Dam Project, India Pushes Forward
In the past, threats against the Sapta Kosi project have caused surveillance work in the area to be suspended repeatedly. But after the latest round of warnings, the Nepalese government adopted a different tactic, pledging heightened security in the region to ensure the safety of Indian officials doing fieldwork.
In doing so, Nepal’s coalition government is throwing its limited weight around, and—to a degree—staking its reputation on its ability to prevent an outbreak of violence. Historically, Nepal’s government has been largely bypassed or ignored in matters of hydroelectric development. As Nepal Water Conservation Foundation Director Dipak Gyawali told International Rivers in a June 2010 interview:The main players are private investors, with state entities and civil society unable to stand up to them….In Nepal, we just saw local politicians burn down the office of an international hydropower company even after the project was sanctioned by their leaders in the central government.
Gyawali added that during the Nepalese civil war (1996-2006), private developers were able to build “small hydropower projects even while a Maoist insurgency was raging because they did not ride roughshod over local concerns.” Regarding Sapta Kosi, Gyawali said the government should adopt a similar approach, and “start listening to the marginalized voices.” Otherwise, he warned, the Indian-Nepalese team spearheading the project “will be faced with delays, impasse, and intractable political problems,” including the potential for Maoist violence in the region. (As noted earlier this month in New Security Beat, the Indian government has also struggled with Maoist-linked violence in recent years, as New Delhi struggles to pacify a Naxalite insurgency in eastern and central India.)
Rural Nepal’s Troubles Far Bigger Than Sapta Kosi
Maoists may be wielding Sapta Kosi as a weapon to gain political leverage both in the countryside and Kathmandu, but the proposed dam is far from the only environmental issue impacting rural lives and threatening to undermine support for the central government.
In a country where firewood still accounts for 87 percent of annual domestic energy production, deforestation has been hugely problematic across rural Nepal. As of 2010, less than 30 percent of the country’s original forest-cover now remains. The rapid removal of forest cover has reduced soil quality, exacerbated seasonal flooding, and caused degraded water quality due to high sedimentation levels.
Further, as the country’s population grows at an annual rate of 2 percent, low soil productivity and unsustainable farming practices have turned Nepal’s effort to feed itself into a constant uphill struggle. According to the World Bank, the country sports one of the world’s highest ratios of population to available arable land, paving the way for potential further food shortages.
Sustainable energy development in Nepal perhaps represents one way of slowly restoring environmental health to the country. By investing in a more reliable national power grid, the central government could reduce rural dependence on firewood for fuel, allowing the country’s forests, soil, and waters to recover even as population increases. Further, hydroelectric projects like Sapta Kosi—implemented with greater involvement from local communities—could play an important role in moving the country forward. With an estimated untapped hydroelectric potential of 43,000 megawatts, Nepal could not only meet its own energy needs by developing its waterways, but profit from hydroelectric energy exports as well.
On the other hand, the Nepalese government could—at its own peril—continue to overlook rural populations’ grievances, and the environmental degradation unfolding outside Kathmandu. If left unchecked, however, these conditions could once again make the Maoist insurgency an appealing movement, potentially reviving grassroots support for anti-government extremism.

In Pakistan, Clinton Calls for Human Security; USAID’s Shah Commends Birth Spacing
›July 20, 2010 // By Russell SticklorIn Islamabad yesterday, Secretary of State Hillary Clinton acknowledged longstanding Pakistani concerns that the U.S.’s ongoing mission in the country is solely military in nature. However, Clinton asserted at the opening of the second U.S.-Pakistan Strategic Dialogue that the “future demands a comprehensive human security, a security based on the day-to-day essentials like jobs, schools, clinics, food, water, fuel, equal access to justice, [and] strong, accountable public institutions.” To that end, she announced a $500 million assistance package earmarked largely for new agricultural and hydroelectric infrastructure development, as well as the construction of new hospitals and other health infrastructure.
Family planning was another key element in this week’s U.S-Pakistani talks. A U.S. delegation headed by USAID Administrator Rajiv Shah met with top Pakistani health officials to discuss the strategic importance of birth spacing. Both sides agreed that encouraging women to extend the interval between bearing children would not only improve maternal and child health, but also start to bring Pakistani’s population growth rate down to a more sustainable level—a goal fully explored at a recent Wilson Center conference on Pakistan’s population challenge.
As Pakistani demographer Zeba Sathar told New Security Beat in an interview at the conference, educating young women and empowering them to control their own reproductive health will allow them to “take care of their fertility and their family size themselves”—a development that could ease Pakistan’s resource crunch and reduce traditional gender inequities in the years to come.

New Film Looks at Sub-Saharan Africa’s Unmet Need for Family Planning
›A new documentary film released recently by Population Action International brings attention to the plight of women across sub-Saharan Africa who lack access to basic reproductive health supplies, such as condoms and contraceptives. Funded with the support of the Reproductive Health Supplies Coalition, “Empty Handed” documents the unmet need for family planning services in the region, which has some of the world’s highest fertility rates.
PAI filmmaker Nathan Golon shot the film in Uganda earlier this year. The film’s focus on Ugandan women’s struggles in particular is with good reason, as the country has a well-documented history of providing insufficient family planning services. According to the CIA’s World Factbook, Uganda has the world’s second highest total fertility rate at 6.73 children per woman.
“Empty Handed” examines how a lack of family planning tools and services can lead to a slippery slope of unintended consequences, from unplanned pregnancies to the rampant spread of sexually transmitted diseases. The film revolves around interviews conducted with women who share common hardships as they try to access family planning from under-resourced local healthcare clinics, often traveling long distances only to find upon arrival that no contraceptives or condoms are available.
In addition to identifying past and current issues with reproductive healthcare access in sub-Saharan Africa, “Empty Handed” also puts forward some ideas for better meeting family planning needs of the more than 200 million women throughout the world without access to even basic contraception.

USAID Head Calls for Integrating Health Services in New Global Health Initiative
›July 2, 2010 // By Russell SticklorThis Tuesday, Dr. Rajiv Shah, administrator for U.S. Agency for International Development gave a major speech at the Center for Strategic & International Studies on USAID’s Global Health Initiative. With $63 billion earmarked for GHI over the next six years, there are high expectations for the program.
Shah laid out some details of GHI’s main priorities, which include improving family planning services, enhancing nutrition initiatives, and building stronger and broader-based healthcare systems across the developing world, with special attention paid to the health needs of mothers and their young children. He offered a number of examples of the benefits of integrating family planning with other health services for women and children, including maternal health and nutrition.
Shah did not, however, mention integrating family planning with environmental programs, the benefits of which USAID-funded programs have amply demonstrated. He also did not delve into the emerging nexus of family planning, population growth, and climate change, a subject of much discussion at last month’s Women Deliver conference in Washington, D.C.
On the other hand, Shah did say that GHI’s emphasis on improving nutrition for the world’s poor will complement another major Obama administration outreach effort, the Feed the Future initiative—repeating a point he made at the recent launch of the food security effort.
Shah also highlighted the need for establishing benchmarks for measuring success that revolve around people, not diseases. He suggested one way of achieving this would be to ensure that clinics—particularly in rural areas—broaden their mandate to offer a variety of health-prevention services, rather than providing resources that treat primarily one type of illness.

‘Interview:’ Educate Girls, Boys, To Meet the Population Challenge, Say Pakistan’s Leading Demographers
›June 25, 2010 // By Russell SticklorPakistan is at a major demographic crossroads. With a youth-heavy population of some 180 million and an annual population growth rate around 2 percent, the country’s population is projected to swell to roughly 335 million by mid-century. Such explosive growth raises major questions about Pakistan’s future, from looming food and water scarcity, to yawning inequalities in the nation’s educational and economic systems. Some warn that these problems threaten to drive a new generation of disaffected Pakistani youth toward political and religious radicalization.
Recently, I asked a number of leading Pakistani demographers visiting the Wilson Center how their country could best achieve more sustainable population growth rates and effectively harness the economic potential of Pakistani youth.
Educate Girls
Zeba Sathar, Pakistan country director for the Population Council in Islamabad, told me empowering girls through education represents one of the most important means of reducing the total fertility rate, which currently stands at four. “When children are educated—particularly when girls are educated—they take care of their fertility and their family size themselves,” Sathar said.
Yet securing educational opportunity for Pakistani girls has often been an uphill battle, largely due to entrenched social norms. Yasmeen Sabeeh Qazi, Karachi-based senior country adviser for the David and Lucile Packard Foundation’s Population Program in Pakistan, pointed out that a traditional preference for male children in Pakistani society has meant girls do not often receive the same level of family resources as males. This phenomenon has historically fed gender inequality, she said.
“Since boys are preferred, girls are not given the same kind of attention and nutrition, especially in the poorer and less-educated families,” Qazi told me. “But as the education level goes up, you see that this divide starts narrowing.” For Qazi, one of the keys to heightening educational access for girls is to increase both the quantity and quality of schools in rural districts, where two-thirds of Pakistanis live.
Decentralize Family Planning Services
Others I spoke with emphasized improving access to reproductive health and family planning services in rural and urban areas. The federal government’s relatively recent move to delegate operational authority for family planning services to the provincial or district level has encouraged many, such as Dr. Tufail Muhammad with the Pakistan Pediatric Association’s Child Rights & Abuse Committee in Peshawar.
Muhammad said the ongoing decentralization—which he described as “a major paradigm shift”—is meant to increase ownership of population planning policies at the local level, and therefore lead to more effective implementation because “the responsibility will be directly with the provincial government.” Once capacity is established at the local level to design and implement those policies, Muhammad added, “supervisors, administrators, and policymakers will be very close to [family planning] services.”
Manage the “Youth Bulge”
Yet despite the fact that both the public and private sectors are taking steps to address Pakistan’s growth issues, the population crunch will intensify before it potentially eases. Even assuming that greater educational opportunity for girls and increased local control over family planning services help drop Pakistan’s total fertility rate, the sheer size of the current youth bulge—two-thirds of the country’s population is under age 30—means population issues will inform every aspect of Pakistani society for decades to come.
In discussing the current state of Pakistan’s education system, some speakers also asserted that the influence of madrassas, or religious schools, over the student-aged population has often been overstated. Shahid Javed Burki, Pakistan’s former finance member and recent senior scholar at the Wilson Center, said during the panel discussion that inaccurate enrollment estimates created “the impression that the system is now dominated” by those institutions. He noted that more recent figures place madrassa enrollment at just five percent of the total student population.
With a majority of Pakistani students enrolled in regular public schools, he insisted that “public policy in Pakistan has to focus on public education” in order to prevent those students from slipping through the cracks. The problem, he added, was that “the education that they are receiving is pretty bad. It really does not prepare them to be active participants in the workforce and contribute to the economic development of the country.”
Still, most of the experts sounded optimistic about Pakistan’s potential to mitigate some of the adverse effects associated with the near-doubling of the country’s population during the next 40 years. By starting to more openly address an issue that the government and the powerful Pakistani media have long preferred to ignore, they seemed to agree the country is taking a big step in dealing head-on with its looming demographic challenges.
“I don’t look at the Pakistani population as a burden, but rather as an asset,” remarked Burki. “But it has to be managed.”
